By filling out and digitally signing this form you authorize all repair work along with the materials needed. You and your employees may operate the vehicle for a diagnosis, inspection, or delivery at my risk. A mechanic’s lien is acknowledged on the above vehicle to secure amount of repairs and storage thereto I agree to pay all collection costs associated with this work. It is understood that you and your employees will not be held responsible for loss or damage to the vehicle or articles left in the vehicle in case of fire, theft, or any other cause beyond our control. Sales of all custom order parts are final when ordered and cannot be returned unless defective. You may SMS text message my phone for business purposes.

Vehicles left 3 days after repairs are subject to a $15.00/Day storage fee. Payment is due at time of service.
